Clomid (Clomiphene Citrate): The Honest 2026 Guide
It's been the go-to PCT drug for over 20 years. It works - but the side effects have pushed the community toward better options. Here's the full picture.
What Is Clomid?
Clomid is the brand name for Clomiphene Citrate, the "original" PCT medication. It's been used in the bodybuilding community for decades and has a high clinical success rate for restarting testosterone production and inducing ovulation (its original medical purpose).
However, there's a catch that the modern community has become very aware of: Clomid is a mixture of two isomers. Enclomiphene is the beneficial one. Zuclomiphene is the problematic one. And that distinction makes all the difference.
The Zuclomiphene Problem
This is the core issue with Clomid and the reason it's declining in popularity. Here's what zuclomiphene does:
- Estrogenic activity: While enclomiphene blocks estrogen receptors, zuclomiphene can actually activate them in certain tissues, particularly in the brain. This is counterproductive during PCT.
- Long accumulation: Zuclomiphene has a much longer half-life than enclomiphene and can linger in the body for weeks, continuously exerting its estrogenic effects even after you stop taking Clomid.
- Emotional side effects: The "Clomid Crazies" - mood swings, irritability, emotional sensitivity, anxiety - are primarily attributed to zuclomiphene's activity in brain tissue.
- Visual disturbances: Blurred vision, floaters, and light sensitivity are known Clomid side effects largely tied to the zuclomiphene component.
How It Works (Mechanism of Action)
The basic mechanism is the same as other SERMs: block estrogen receptors at the pituitary gland, which causes the body to release more LH and FSH, which stimulate the testes to produce testosterone.
Clomid is actually very effective at this. Clinical studies show robust increases in LH, FSH, and testosterone levels. The problem isn't that it doesn't work - it works well. The problem is that the zuclomiphene isomer adds side effects that don't need to be there, now that pure Enclomiphene is available.

Clomid has the highest side-effect burden of the three main PCT SERMs:
